Many of Dr. John Owsley's patients travel great distances, often from locations around the world, to have him perform their secondary facelift surgery at his San Francisco Bay Area offices. Since the benefits of a primary (first) facelift do not include stopping the hands of time — just turning the clock back — these individuals are able to maintain their younger, firmer contours with a secondary facelift. Even the best-performed facelift provides only temporary benefit — the aging process continues, with gradual reappearance of the signs of aging that were addressed by the primary facelift. A secondary facelift is a common procedure, performed to regain or maintain the benefits of the first surgery. It may involve the same features as the first facelift, or it may be different, depending on the individual's unique facial characteristics and aging process. An additional procedure may also be appropriate at this time, such as a blepharoplasty, forehead lift, or cheek/chin implants.
In performing a traditional facelift, the surgeon merely pulls back the facial skin to incisions made around the ears. Patients who have a traditional facelift (which is still the most widely performed technique in this country) can expect to see a recurrence of the signs of facial aging after a relatively short period, usually making a secondary facelift desirable within five years of the first facelift.
Dr. Owsley performs secondary facelift surgery at his San Francisco practice for patients who have had "skin-only" primary facelifts performed by other surgeons. In fact, it was the rather brief (five-year) duration of the traditional skin-only facelifts that prompted Dr. Owsley to seek a better, longer-lasting facelift — and he created the SMAS-platysma facelift as the answer.
In Dr. Owsley’s experience, patients who undergo a SMAS-platysma facelift enjoy the aesthetic results for 10-15 years on average before they seek a secondary facelift. This is a significant improvement over the average five years of beneficial results provided by traditional face lift techniques.
Dr. Owsley has performed many secondary SMAS-platysma facelift procedures at his Bay Area practice. These secondary facelifts are combined with a malar fat pad lift. There are only minor differences in the techniques used for the primary and secondary facelift procedures. The incisions are made on the scars from the first facelift surgery, and care is taken so that there is no distortion of the patient’s hair pattern.
In some ways the presence of scar tissue from the previous surgery makes the secondary facelift procedure easier, with less bleeding, and less swelling during recovery. Typically, patients return to normal activities somewhat sooner than they did after the primary operation.
